今日は、大阪市の場合の助成金申請から受け取りまでの流れをご紹介します。

大阪市の場合は、がん治療によるアピアランス（外見）ケアの一環として、
人工乳房エピテーゼには5万円
乳房パッドには3万円
の助成金が申請できます。

必要書類1. 申請書兼請求書

大阪市の助成金申請のページからダウンロードして、記入します。

必要書類2. 本人確認書類の写し

マイナンバーカードや運転免許証のコピーを取りましょう

必要書類3. がん治療を証明する書類の写し

何年前のものでも、手術を受けた当時の書類で病名や術式が書かれているものがあればそのコピーでOKです。

書類がない場合は病院に新たに作成してもらいましょう（有料の場合が多いです）

必要書類4. 乳房エピテーゼ等を購入した領収書の原本

こちらは当店で発行しています。

Subsidy Details for Osaka City

In Osaka City, as part of appearance care (appearance improvement) for cancer patients:

• Artificial Breast Epitheses: Up to ¥50,000
• Breast Pads: Up to ¥30,000

You can apply for this subsidy following the steps below.

Step 1: Prepare Five Required Documents

There are four main steps to the subsidy process, and the most crucial is gathering the required documents. Completing this step means you're nearly done with the application.

Although it may feel a bit tedious, gathering the necessary documents is essential.

Required Documents:

1. Application and Claim Form
   Download this form from the Osaka City subsidy page and fill it out.If you visit our store, we can provide this form directly.Tip:
   Be sure to write a phone number where you can be easily reached, as any discrepancies in the application may prompt a follow-up phone call. It's also a good idea to take a photo of the completed form with your smartphone for your records.
2. Copy of Personal Identification
   A copy of your My Number Card (Individual Number Card) or driver's license is required.
3. Proof of Cancer Treatment
   A copy of any document that shows the diagnosis and surgical details of your cancer treatment will suffice, even if it is from years ago.If you no longer have this document, you can request a new one from your hospital (note that fees may apply).
4. Original Receipt for Your Purchase
   A receipt showing the purchase of artificial breast epitheses or breast pads is required. This must be the original document, not a copy, and should clearly indicate the items purchased.Receipts issued by our store fulfill this requirement.
5. Bank Account Details
   Provide a copy of your bank book or cash card showing the account where the subsidy should be deposited.

Step 2: Submit the Documents by Post

Once you've prepared all five documents, mail them to the designated address. If any issues arise, you will be contacted by phone.
